Metabolic syndrome (MetS) affects over 25% of adults globally, yet diagnosis in rural areas is hampered by lack of laboratory facilities. Simplified non-invasive tools are needed, but their positive predictive value (PPV)—the probability that a positive screen truly indicates disease—must be established to avoid overtreatment. The present study (registry ID PPV-3125926) evaluates a novel 3-item questionnaire’s PPV against reference clinical criteria.

Diagnosis of MetS by International Diabetes Federation/National Heart, Lung, and Blood Institute (IDF/NHLBI) criteria requiring waist circumference plus ≥2 of: triglycerides ≥150 mg/dL, HDL <40/50 mg/dL, BP ≥130/85 mmHg, fasting glucose ≥100 mg/dL (lab-confirmed). ppv 3125926
