Kaneki wakes up in a hospital, confused and disoriented. He soon discovers that his body has undergone a transformation: his eyes have turned into those of a ghoul, and his body is now much stronger.

The story takes place in an alternate version of modern-day Tokyo where ghouls, supernatural creatures that feed on humans, live among humans in secret. The episode introduces Ken Kaneki, a young man who is turned into a half-ghoul after a tragic accident. He is rescued by a group of ghouls and taken to their hideout, where he learns about the world of ghouls.
